International & Domestic Pet Travel Appointment Information
To assure your pet receives the required documentation needed for traveling, please review the important information listed below:
Before booking your Towne Line Veterinary Group Appointment for a Travel Health Certificate, please visit the USDA Health Pet Travel webpage to obtain the documents required for your travel location. Please know, we CAN NOT complete a Travel Health Certificate for your pet without this information. All paperwork must be printed and brought in with your pet on the day of their visit,
Once you have obtained the information needed for your pet per the USDA guidelines, please contact the office to make an appointment. Some locations require these appointments to be made within a specific window of departure, so confirm you have an appropriate date for your visit with us. All previous medical records, vaccine history and microchip information must be provided at the time of the visit.
Travel Appointment Consultations require a large amount of information, so will typically take 45 minutes to 1 hour based on destination application. The Cost of Travel Certificate appointments are: $350 + Office Visit for International Travel Certificates. $85 + Office Visit for Domestic Travel Certificates, not including any additional requirements needed before travel.
Towne Line Veterinary Group does NOT provide online Health Certificate Application Submissions. All applications will be provided to you, the owner, to be submitted at the completion of your visit.
